Forms to Request Records
Below you will find links to the forms you need to request copies of police department records. These forms are in a PDF format and you will need at least version 6 of the Acrobat Reader in order to use them. If the forms do not work or you have an earlier version of Reader please click here to download the latest version.
Make sure that your email application (Outlook Express, Thunderbird, Etc) is running before submitting the form. You can also email a copy to yourself for your records.
If you use a web-based email (Hotmail, GMail, Etc.) you will need to save a copy of the form and attach it to an email. You can also email a copy to yourself for your records.
If you are not able to email the form you can print out a copy and either mail it to us or drop it off.
